Reliant Care Management Company, LLC and the facilities it manages are dedicated to providing quality care in a caring and dignified manner predominantly to both the elderly geriatric population and to those eighteen years of age and older who are afflicted with mental illnesses in addition to a primary medical diagnosis. Our goal is to provide unparalleled care using a team-like approach through empowering employees while at the same time meeting the needs of the respective communities, physicians, hospitals, and other healthcare providers. The objective will be accomplish while working within sound financial practices. Our rehab unit provides quality long term care and/or short term care for individuals with complex medical and skilled medical illnesses. Our rehab unit is designed to meet the needs of the resident's with the primary diagnoses of complex medical or skilled medical needs with ancillary diagnosis of mental illness.
