Reliant Care Management Company, LLC and the facilities it manages are dedicated to providing quality care in a caring and dignified manner predominantly to both the elderly geriatric population and to those eighteen years of age and older who are afflicted with mental illnesses in addition to a primary medical diagnosis. Our goal is to provide unparalleled care using a team-like approach through empowering employees while at the same time meeting the needs of the respective communities, physicians, hospitals, and other healthcare providers. The objective will be accomplish while working within sound financial practices. SPECIAL CARE UNITS
Medicare unit
A variety of skilled services including physical, occupational and speech therapies are available as well as IV therapy. Since all of our beds are dual certified to include Medicare we are staffed with licensed professionals 24 hour a day. Our goal is to assist our residents in returning to their home, functioning as independently as possible.
Physical rehabilitation unit
At Levering Regional Health Care Center we have a rehab unit in response to the community needs: quality long term care and/or short term care for individuals with complex medical and skilled medical illnesses. Our rehab unit is designed to meet the needs of the resident's with the primary diagnoses of complex medical or skilled medical needs with ancillary diagnosis of mental illness.
Residential care unit
This unit is for the resident who does not necessarily require heavy nursing care. It has an 35 bed unit designed in an "home like" setting. The residential care unit offers your choice of semi-private, private or deluxe accommodations. Nursing supervision is available to monitor, medications and to provide care if needed.
